For nearly four decades, I watched people do everything they were told to do.

Yet many experienced the same exhausting pattern: progress followed by setbacks. Relief followed by recurrence. Effort without lasting stability.

In my role as a state-appointed medical examiner, I examined case after case and watched the same pattern repeat across widely different situations. Many people improved. But improvement often failed to hold.

That gap gradually shifted my attention away from the response itself, and toward what people were repeatedly experiencing in between.

The question was no longer simply what can change the response? I began looking instead at what the response might be telling us.

When the conditions around people stop supporting stability, protection begins to appear.

For an individual, it may look like pain, hesitation, recurring setbacks, or a quiet loss of trust in the body. Across an organization, it may appear as friction, caution, implementation fatigue, resistance, or people working harder while accomplishing less.

These responses are often interpreted as failure. They may instead be evidence of what people have been adapting to.

When the conditions that make protection necessary remain unchanged, progress becomes difficult to sustain, no matter how hard the effort.

When conditions continue to make protection necessary, progress struggles to hold. When conditions become more supportive, a different response and greater stability become possible. That is the shift. We stop viewing the response as an isolated problem and begin recognizing it as evidence.

What began as an observation about human adaptation became a discipline about the conditions that shape it.

And too often, that progress did not hold.
